One of the most common concerns I hear from patients is, “Everyone keeps asking me if I’m tired.” Even after a full night’s sleep, hollow under-eyes, dark circles, fine lines, and drooping eyelids can make you appear older, stressed, or exhausted.
The good news is that there are many effective ways to rejuvenate the eye area. The key is choosing the right treatment—or combination of treatments—for your unique facial anatomy. At my practice in St. Petersburg, Florida, I believe every patient deserves a personalized treatment plan designed to achieve natural, refreshed results that enhance—not change—their appearance.
What Causes Tear Troughs and Tired-Looking Eyes?
The tear trough is the natural groove that runs from the inner corner of your eye toward the upper cheek. While everyone has this anatomical feature, it often becomes more noticeable with age due to several factors:
- Loss of facial fat and volume
- Decreased collagen and elastin production
- Thinning skin beneath the eyes
- Descent of the cheeks over time
- Genetics
- Sun damage and lifestyle factors
In addition, the upper eyelids may begin to droop, excess skin can develop, and fat pockets beneath the eyes may become more prominent. These changes often work together to create a tired appearance.
Many patients believe they have “dark circles,” when in reality they are seeing shadows created by volume loss or puffiness beneath the eyes.
During your consultation, I carefully evaluate your facial structure, skin quality, muscle activity, eyelid position, and overall facial balance before recommending treatment. There is never a one-size-fits-all solution.
Dermal Fillers: Restoring Lost Volume
For many patients, hyaluronic acid fillers are one of the most effective non-surgical treatments for tear troughs.
Rather than simply filling a hollow, my goal is to restore a smooth, youthful transition between the lower eyelid and the cheek. When performed conservatively and precisely, filler can soften shadows while maintaining a completely natural appearance.
Benefits of Tear Trough Fillers
- Immediate improvement
- Minimal downtime
- Restores lost volume
- Softens hollow under-eyes
- Reduces the appearance of dark circles caused by shadows
- Creates a refreshed, well-rested look
Results typically last 9 to 18 months, depending on the individual.
The under-eye area is one of the most technically demanding places to inject filler. Precision is everything, which is why I take a conservative approach using advanced techniques that prioritize safety and natural-looking results.
Botox®: Softening Wrinkles Around the Eyes
Patients are often surprised to learn that Botox does not fill tear troughs—but it can play an important role in overall eye rejuvenation.
Botox relaxes the muscles responsible for dynamic wrinkles and can help:
- Soften crow’s feet
- Smooth fine lines around the eyes
- Create a subtle brow lift
- Reduce excessive muscle movement that contributes to facial aging
When combined with dermal fillers, Botox creates a balanced, refreshed appearance without looking overdone.
Upneeq®: A Non-Surgical Lift for Droopy Eyelids
Not every patient who feels they look tired is bothered by under-eye hollows alone. A slightly drooping upper eyelid can make the eyes appear smaller, heavier, and less alert.
Upneeq® is the first and only FDA-approved prescription eye drop designed to temporarily lift the upper eyelid in adults with acquired ptosis (low-lying eyelids). By stimulating the muscle responsible for elevating the eyelid, Upneeq can provide a noticeable lift in as little as 15 minutes, with results lasting approximately 6 to 8 hours.
Many patients appreciate Upneeq because it can:
- Open the eyes for a brighter appearance
- Improve mild eyelid drooping without surgery
- Enhance eyelid symmetry
- Improve the upper visual field in some patients
- Pair beautifully with other aesthetic treatments
For patients with mild eyelid drooping, Upneeq can be an excellent non-invasive option or even a way to “test drive” a more open-eyed appearance before considering surgery.
PDO Threads: Lifting While Stimulating Collagen
For patients experiencing early skin laxity or mild descent of the cheeks, PDO threads can provide improvements while stimulating the body’s natural collagen production.
PDO threads are absorbable sutures placed beneath the skin to gently reposition tissue and improve structural support.
Benefits include:
- Mild lifting of the cheeks
- Better support for the under-eye area
- Firmer skin
- Increased collagen production
- Natural-looking rejuvenation with little downtime
PDO threads are often an excellent option for patients who want more support than filler alone but are not yet ready for surgery.
Blepharoplasty: When Surgery Is the Best Solution
While non-surgical treatments such as fillers, Botox®, PDO threads, Upneeq®, and medical-grade skincare can produce beautiful improvements, some patients have concerns that are best addressed with surgery.
Blepharoplasty (eyelid surgery) remains the gold standard for correcting excess skin, prominent fat pockets, and significant eyelid aging. Depending on your anatomy and goals, I may recommend an upper blepharoplasty, lower blepharoplasty, or a combination of both.
Upper Blepharoplasty
As we age, the upper eyelids can become heavy due to excess skin and loss of elasticity. This can create a tired appearance and, in some cases, even interfere with peripheral vision.
An upper blepharoplasty removes excess skin and, when appropriate, small amounts of fat to restore a brighter, more youthful appearance.
Benefits include:
- Opens and brightens the eyes
- Removes heavy, sagging upper eyelid skin
- Restores a refreshed appearance
- May improve peripheral vision
- Long-lasting, natural-looking results
While Upneeq is an excellent option for mild eyelid drooping, surgery provides the most effective long-term correction when excess skin is the primary concern.
Lower Blepharoplasty
The lower eyelids often show aging through puffiness, protruding fat pads, loose skin, and deeper tear troughs.
A lower blepharoplasty allows me to carefully reposition or remove excess fat, tighten the lower eyelid when appropriate, and create a smoother transition between the eyelid and the cheek.
Benefits include:
- Reduces under-eye bags
- Smooths puffiness
- Improves lower eyelid contour
- Creates a more rested appearance
- Long-lasting rejuvenation
Healthy Skin Matters
Even after correcting volume or tightening tissue, healthy skin remains one of the most important components of eye rejuvenation.
One of my favorite products for the delicate eye area is Obagi ELASTIderm® Eye Cream.
This advanced eye treatment helps:
- Improve skin elasticity
- Reduce fine lines and wrinkles
- Firm crepey skin
- Hydrate the delicate eye area
- Support healthy collagen and elastin
I often recommend ELASTIderm before and after aesthetic treatments because healthy skin enhances every result.
Why Combination Treatments Often Produce the Best Results
The eyes age in many different ways. You may have volume loss, wrinkles, drooping eyelids, skin laxity, and thinning skin—all at the same time.
Because of this, the most beautiful results often come from combining treatments.
Your customized treatment plan may include:
- Hyaluronic acid fillers to restore lost volume
- Botox® to soften wrinkles and gently lift the brows
- Upneeq® to temporarily lift mildly drooping eyelids
- PDO threads to support tissue and stimulate collagen
- Obagi ELASTIderm® Eye Cream to improve skin quality
- Upper blepharoplasty to remove excess upper eyelid skin
- Lower blepharoplasty to correct under-eye bags and smooth the lower eyelids
No two patients age exactly alike, which is why every treatment plan I create is individualized.
My Philosophy
My philosophy has always been simple: enhance, never change.
Whether I’m performing surgery or a non-surgical treatment, my goal is to help you look refreshed, healthy, and confident while preserving what makes you uniquely you.
As both a board-certified plastic surgeon and aesthetic injector, I can offer the full spectrum of eye rejuvenation—from medical-grade skincare and injectables to advanced eyelid surgery. This allows me to recommend the treatment that’s truly best for you, rather than a one-size-fits-all approach.
Every face tells a story, and every treatment should respect your natural beauty. During your consultation, we’ll discuss your concerns, evaluate your anatomy, and develop a personalized treatment plan tailored to your goals, lifestyle, and comfort level.
If you’ve been bothered by tired-looking eyes, dark circles, under-eye hollows, drooping eyelids, or excess skin, I’d love to help you explore your options. Together, we can determine the treatment—or combination of treatments—that will help you achieve brighter, more youthful eyes while still looking completely like yourself.
